Known Accessibility Issues and Evaluation Gaps

A11Y-001 — Native visual-audit diagnostics
Status: unresolved diagnostic findings. Some report elements produce Dynamic Type findings despite using semantic fonts. Contrast and element-detection findings also remain for review, including system materials and composed presentation states. Existing palette tests do not prove that every rendered pixel combination passes.
Next evaluation: reproduce the relevant screen on an existing iPhone configuration at the affected text size or appearance, determine whether there is an actual usability failure, and fix confirmed barriers. Retain diagnostics until they are explained or resolved.
A11Y-002 — Assistive-technology task completion
Status: evaluation incomplete. Automated accessibility hierarchy and action checks have passed for the covered iPhone flows. They do not establish VoiceOver focus order, spoken clarity, Voice Control operation, braille or hardware-keyboard navigation, or Switch Control task completion.
Next evaluation: complete the affected core tasks with the relevant technology, including navigation, data entry, validation errors, retry, and cancellation. Prioritize barriers that prevent using the record.
A11Y-003 — System interfaces and exported reports
Status: evaluation incomplete. Physical-device authentication, notification interactions, Siri/Shortcuts, privacy covers with a sheet open, and share destinations need end-to-end checks. Health authorization/import is deferred; the current app does not request Health access. PDF structure and text extraction are tested; assistive-technology behavior across PDF readers remains unverified.
Current alternatives: core record entry does not require notification permission, and Health import is off. Reports offer a complete text alternative alongside PDF. External system interfaces still need their own task evaluation.
A11Y-004 — Device and OS coverage
Status: limited coverage. Routine validation is focused on iPhone. A complete iPad, multitasking, landscape, and older-supported-OS matrix has not been completed. The app’s deployment target or device-family declaration is not proof of accessibility coverage on those configurations.
Next evaluation: test only the configurations needed for a specific change during development; complete applicable platform evaluation before advertising broader support. No ongoing iPad test run is part of routine development at present.
Reporting and resolution
Open a barrier report and mention an identifier above if relevant. Include only non-sensitive reproduction information. Resolved items will retain their history and identify the affected app version and supporting verification; there is no guaranteed repair date.
